fwd collective is a team of social work practitioners and support coordinators who collaborate to provide high-quality, person-centered support. We specialise in working alongside people with psychosocial disability, using trauma-informed and recovery-oriented approaches to guide our work. Each team member operates independently under their own ABN while sharing key operational processes to ensure comprehensive and consistent services for clients.

We welcome referrals for NDIS Support Coordination and a range of social work services, including project-based work. To find out how we can best support your needs, please visit our referrals page or contact us at hello@fwdc.com.au.

At fwd collective, our vision is to create new pathways for both clients and practitioners—fostering growth, collaboration, and meaningful outcomes.

Who we are

fwd collective support coordinators operate as independent business owners and are responsible for managing their own billing, workload, accounting, and related administrative functions.

All support coordination services are delivered by the individual support coordinator engaged under your service agreement. Your primary coordinator is solely responsible for providing services, maintaining records, and managing all client-related communication.

If you would like continuity of support, we have the option to link you in with another secondary support coordinator within the fwd collective brand. This arrangement is entirely optional and requires a separate service agreement with that coordinator. The secondary coordinator will only provide services where formally engaged and agreed.

Each team member invoices in 15-minute increments and maintains client records in a safe and secure manner in accordance with privacy and professional obligations. Team members operate independently while adhering to shared operational policies and procedures to ensure consistent practice standards across the collective.

All fwd collective team members share common professional values and are committed to upholding the Australian Association of Social Workers (AASW) Code of Ethics and Practice Standards.
